    • Job Requirements
    • Must be AHA BLS Certified.
    • Nursing only Michigan hospitals need their licensure verified through LARA not NURSYs.
    • Must be AHA ACLS Certified.
    • Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals.
    • Completed prior to start and updated every 3 years: Ascension Color Vision.
    • Must complete HIPAA, Joint Commission -NPSG, OSHA Topics.
    • NIH Certification required.
    • Non Violent Crisis Intervention Training. Required to be an 8 hr course.
    • Exam and answer key found in Document Library. Agency is required to give and grade the test. Score needs to be 90% or higher with only 2 attempts.
    • Optional for Onboarding, but needs to be uploaded by our Ascension unit leader within a week: Ascension Unit Specific Checklist.
    • www.ascension.org/CWRnursingorientation - The attestation at the end of the pre-hire 6 hours needs to be uploaded by the Agency.
    • Done onsite: Ascension Nursing Contract Worker Onboarding.
    • Instructions in the Document Library: Ascension Nova StatStrip.
    • Vaccines or Positive IGG Titer required.
    • Annual Fit Test required.
    • Documentation of vaccination series completion or documentation of declination to vaccine for Hepatitis B.
    • Flu season starts Sept 4th, 2023 for all new travelers. Needs to be updated every year. Declinations and exemptions not accepted.
    • X-ray completed for Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire.
    • Statement of good health or completed physical exam prior to first assignment, updated annually thereafter.
    • Required for all travelers. If placement is in a state other than the home state/issuing state of compact license for RNs, address on Driver's license must match the home state for the compact license.
    • 1st within 1 year, 2nd within 30 days of start. And/or Negative IGRA within 90 days of offered start date lab printout prior to start.
    • The candidate or associate should provide documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider.
    • Valid 10 years from the administered date: Tdap.
    • Qualifying reference(s) completed prior to first assignment. At least two professional references.
    • Include Social Security Trace, Employment Verification, OIG Search and National Sex Offender.
    • Clinical only; Required annually in specialty: Ascension Skills Checklist.
    • Within 30 days prior to the start of the first assignment with the client, updated annually thereafter. If break in service > 90 days, retesting is required: Ascension 12 Panel Drug Screen.
    • Education needs to be validated at the source level in accordance with applicable governing laws.
    • You may upload your standard COA, Ascension does not require their specific form: Ascension Confirmation of Assignment.
    • Flu season is October 1, 2024 through March 31, 2025. Exemptions/Declinations are accepted, but candidate will be required to wear a mask at all times: Ascension Influenza Vaccine Exemption/Declination.
